CPNI Info and Authorization

By authorizing your telecom consultant access to your CPNI, you authorize him/her to see your invoice and view your call detail. Without such authorization, your telecom consultant will not have access to your detailed invoice and will be limited in the advice he/she can offer regarding your telecom services.

Federal law enables you to protect the confidentiality and limit the use of your CPNI. If you wish to grant your telecom consultant access to your CPNI, please complete the information below.
